A Suspension/Revocation Order and Temporary License Due to DUI is completed when a subject is stopped/arrested and the Law Enforcement Officer believes the subject is under the influence and the subject is required to surrender his/her driver's license. The Law Enforcement Officer reports the incident to the Department of Motor Vehicles. The Suspension/Revocation Order and Temporary License Due to DUI contains driver's personal information vehicle information intoxication symptoms chemical tests and officer's statement. The Suspension/Revocation Order and Temporary License Due to DUI is one of 44 IEPDs developed by the California Department of Justice - Data Exchange Project to support information exchange between state and local government organizations throughout the state of California. It could also be adapted for use in other states.
