Psychiatric Assessment

A Psychiatric Assessment is sent from the Probation Department to the Youth Authority if the Court has sentenced a subject to the Youth Authority and if the subject has had a mental health assessment/psychiatric evaluation.  The Psychiatric Assessment contains medical history mental status diagnosis subject information and signature block.  The Psychiatric Assessment is one of 44 IEPDs developed by the California Department of Justice - Data Exchange Project to support information exchange between state and local government organizations throughout the state of California.  It could also be adapted for use in other states.
