Court Recommendation for Investigation by Department of Motor Vehicles (DS145)

A Court Recommendation for Investigation by Department of Motor Vehicles (DS145) is a form sent from the court to the Department of Motor Vehicles when the court suspects a safety issue and recommends the Department of Motor Vehicles investigate the driving privilege of a subject.  The DS145 contains information about a driver reason for recommending the investigation and signature of a judge or court clerk.  The Court Recommendation for Investigation by Department of Motor Vehicles (DS145) is one of 44 IEPDs developed by the California Department of Justice - Data Exchange Project to support information exchange between state and local government organizations throughout the state of California.  It could also be adapted for use in other states.
